Name Change for Students
Students requesting a legal name change at Yavapai College must provide documentation before the student record can be updated.
How to Request a Legal Name Change
To change your name on your student record, upload a copy of your driver’s license reflecting your new name and additional legal name change documentation.
- Prepare a copy of your driver’s license showing your new legal name.
- Include additional name change documentation, such as a marriage record, divorce record, or court order.
- Upload the documentation using the secure upload link.
- In the “Where should we deliver this document?” section, select Admission.
Your username will be updated as part of the name change process.

Submit your documentation through the secure Yavapai College upload system.
Acceptable Documentation Examples
- Driver’s license reflecting your new name
- Marriage record
- Divorce record
- Court order
Additional Notes
Students who are also YC employees
If you are employed at Yavapai College, including student employees, staff, and faculty, you must also change your name on personnel and/or payroll records with Human Resources on the Prescott Campus. Call 928.776.2217 for more information.
Pronouns and preferred names
Pronouns and preferred names for students, faculty, and staff may be changed in the myYC Experience portal by expanding the navigation ment, selecting the My Account card, then Update My Profile.
